What kind of orbital wall fracture needs to be repaired?
Most orbital fractures will heal well without intervention. Every once in a while, however, the patient needs surgical repair. Findings that would indicate need for surgery include:
- Eye muscle entrapement (signs of restricted movement on exam with forced ductions)
- Eye muscle entrapement with a bradycardic response with eye movement
- Enophthalmos (sunken-in eye)
- Downward displacement of the globe

My one injured eye has been diagnosed as a permanent traumatic mydriasis with a ruptured sphincter muscle. My fully blown pupil never moves or even quivers. The eye specialist wonders why it doesn’t quiver in some position etc,? Isn’t there a difference between a ‘fully’ blown pupil and one not completely blown?
