What is the “ISNT rule” in glaucoma?
The ISNT rule is an easy way to remember how the optic nerve is supposed to look in a normal eye. Normally the neuro-retinal rim is thickest Inferiorly and thinnest Temporally.
With glaucoma, however, you begin to see vertical thinning, with atrophy along the inferior and superior rims. Thus, when optic nerves don’t follow the ISNT rule, they may have had glaucomatous damage.

WHAT IF INFERIOR AND SUPERIOR RIM’S ARE THINNED OUT BUT BOTH EYES HAVE SIMILAR CD RATIO(0.5) . VERTICAL HORIZONTAL DIFFRENCE OF 0.1 ONLY. WITH NO INCREASE IN IOP AND PATIENT HAVING NO MODIFIABLE RISK FACTORS SUCH AS BP, DM, SMOKING, NORMAL LIPID PROFILE MALE 40YR. SHOULD WE STILL GO FOR VF
You need VF for sure. Get good baseline stereo disc photos. OCT exam to evaluate nerve fiber thinning would be helpful to evaluate glaucoma risk also.
Does cataract interfer with making a proper diagnose of glucoma as it related to field vision test? Does HTN, eye pressure or sinsus problems affect the pressure in eyes? When cataracts are removed from eyes will it restore vision and the ability to read? Are there any white people who have glucoma? Can a person have glucoma with no family history? There is a person (African American) who has had high eye pressure since age 30 and is now 73 with no family history. She has yet to be diagnose with glucoma but rather diagnose with ocular htn.
